If you are registered with Disability Services and have been asked to apply for a notetaker for any of your courses, please fill out the application form below.
Please note the following:
Notetakers are provided for lectures only.
We are unable to provide notetakers for seminars, tutorials, lectures based significantly on discussions, online courses, studios or intensive courses that are held over full days (such as Summer Semester or three-day courses).
If you change your course enrolment, or drop a course during semester, please notify Disability Services immediately by email firstname.lastname@example.org. You will need to fill out a request form for each new course you require a notetaker for.
To ensure we recruit a notetaker for the correct courses, we are unable to put the service in place until you submit your form.
